Report it from the phone
Any operative can report a hazard or near miss from their page, with a short description and a photo. It reaches the site manager and the office straight away, so a trailing cable across a fire exit is dealt with in minutes, not at the end of the week.
Clear stages, clear owners
Each report moves through stages that suit its type: a hazard is reported, made safe and fixed; a near miss is reported, looked into and its actions completed. Practical reports such as a full skip or missing materials go to the site manager first.
Chased if left
If a report sits at one stage too long, the office is told. You can set who handles each type of report and how long it can wait before it is escalated.
Kept as evidence
The person who reported it is told each time it moves on, and the full history, with photos and who closed it, is kept as PDF evidence and included in the monthly safety summary.
